![]() Resembling an antique gardening tool, the bass clarinet is not the most grateful instrument for a solo role, limited in range and expression. Only the prominent use of the opening motif of Beethoven’s Moonlight Sonata in the closing section seems a bit twee, distracting one from Musgrave’s own compelling musical argument. Otherwise Autumn Sonata is an effective work, evocative, concise (20 minutes), and smartly scored. Near the end a second offstage bass clarinet echoes the soloist’s line. There is a violently explosive march-like middle section (con furore), followed by a searching Lamentoso. The solo line is closely woven with the undulating mystery of the orchestral fabric. Befitting the concerto’s title, the music is autumnal, restless and dream-like, cast in a dark expressive vein with ominous militaristic shadows in the martial rhythms and percussion writing. Most attention Thursday night focused on Autumn Sonata, a bass clarinet concerto by Thea Musgrave, which was being heard in its belated American premiere.Ĭast in six unbroken sections, the 1994 work takes inspiration from the haunting poems of the Austrian writer Georg Trakl, who died in a Polish hospital during the First World War. With powerful, richly eloquent, and immaculately balanced performances, Mälkki’s sensational debut is easily the most impressive CSO podium bow of recent seasons. Mälkki, music director of Ensemble InterContemporain, managed to not only pull off each work on this demanding program with striking success but sparked the CSO to some of their finest playing of the year. premiere of a bass clarinet concerto, flanked that assignment with two challenging works by Charles Ives, and closed the evening with Richard Strauss’s epic tone poem, Also sprach Zarathustra. Until recently, the term 3D printing has been associated with machines low in price or in capability. The term 3D printing still referred only to the polymer technologies in most minds, and the term AM was more likely to be used in metalworking and end-use part production contexts than among polymer, inkjet, or stereolithography enthusiasts.īy the early 2010s, the terms 3D printing and additive manufacturing evolved senses in which they were alternate umbrella terms for additive technologies, one being used in popular language by consumer-maker communities and the media, and the other used more formally by industrial end-use part producers, machine manufacturers, and global technical standards organizations. In contrast, the term subtractive manufacturing appeared as a retronym for the large family of machining processes with material removal as their common process. The umbrella term additive manufacturing (AM) gained popularity in the 2000s, inspired by the theme of material being added together ( in any of various ways). Fused deposition modeling (FDM), which uses a continuous filament of a thermoplastic material, is the most common 3D printing process in use as of 2020. One of the key advantages of 3D printing is the ability to produce very complex shapes or geometries that would be otherwise infeasible to construct by hand, including hollow parts or parts with internal truss structures to reduce weight. As of 2019, the precision, repeatability, and material range of 3D printing have increased to the point that some 3D printing processes are considered viable as an industrial-production technology, whereby the term additive manufacturing can be used synonymously with 3D printing. In the 1980s, 3D printing techniques were considered suitable only for the production of functional or aesthetic prototypes, and a more appropriate term for it at the time was rapid prototyping. It can be done in a variety of processes in which material is deposited, joined or solidified under computer control, with material being added together (such as plastics, liquids or powder grains being fused), typically layer by layer. 3D printing or additive manufacturing is the construction of a three-dimensional object from a CAD model or a digital 3D model. ![]() ![]() ![]() ![]() Emphasis was given to articles published within the last ten years. ![]() We here review the available literature, using a PubMed search with the following keywords: cerebral blood flow, transcranial Doppler, syncope, orthostatic hypotension, autonomic testing and autonomic failure. We focus on the transcranial Doppler (TCD) as a non-invasive method to evaluate cerebral hemodynamics and its usefulness in the outpatient autonomic clinic. The elderly population, in particular those with autonomic or cardiovascular disorders, are at a greater risk for dementia, stroke, long-term disability and death. The failure of cerebral autoregulation can occur at any age. This adaptability ensures that the delivery of oxygen and nutrients can meet the high metabolic demands of the neuronal tissue in different regions of the brain. This interplay occurs at the level of the arterioles in the cerebrovasculature and at the neurovascular unit, and over multiple time scales from seconds to hours. The CBF autoregulation involves integrative interactions between brain tissue metabolism, systemic blood pressure, arterial blood gases, as well as neurogenic input from the central autonomic network. The inbuilt capacity of the cerebral circulation to regulate its own flow to remain constant in the face of changes in perfusion pressure is known as cerebrovascular autoregulation. When blood flow falls below the critical limit, even for a few seconds, syncope (i.e., a reversible loss of consciousness with no neurological sequelae) occurs. Brain tissue has high metabolic demands and, in order to maintain consciousness, it must receive an adequate supply of blood flow to ensure that energy and oxygen demands are met. Despite the human brain weighing only 2% of the total body mass, it receives 15% of the cardiac output at rest and consumes 20% of the body’s oxygen. We also describe the control of cerebral blood flow in autonomic disorders and functional syndromes.Ĭerebral blood flow (CBF) is normally 50–60 ml/min per 100 grams of brain tissue. We review clinical standards for the acquisition and analysis of TCD signals in the autonomic laboratory and the multiple methods available to assess cerebral autoregulation. Syncope occurs when hypotension is severe and susceptibility increases with hyperventilation, hypocapnia and cerebral vasoconstriction. Cerebral autoregulatory mechanisms help defend the brain against hypoperfusion when perfusion pressure falls on standing. TCD can be very useful in the work up of a patient with recurrent syncope. The transcranial Doppler (TCD) is a non-invasive technique that provides real-time measurements of cerebral blood flow velocity. This involves the interplay between myogenic and metabolic mechanisms, input from noradrenergic and cholinergic neurons, and the release of vasoactive substrates including adenosine from astrocytes and nitric oxide from the endothelium. The cerebral circulation has its own complex finely tuned autoregulatory mechanisms to ensure blood supply to the brain can meet the high metabolic demands of the underlying neuronal tissue. When cerebral blood flow falls below a critical limit, syncope occurs and if prolonged ischemia leads to neuronal death. ![]() |
